The intent of the Service Forms section is to provide you with tools that will enhance your ability to identify potential accessibility and/or accommodation needs of persons with disabilities whom you serve. The City of Los Angeles Workforce Development Division does not mandate nor require you or your WorkSource center to utilize the succeeding forms.
If you are a City of Los Angeles funded WorkSource center, the Disability Program Specialist assigned to your region is available to answer questions or provide training to your staff regarding the effective use of these forms.
